What you need, and when you need it
The requirement list is the Medical Education Commission’s. The notes beside each item are ours — the practical failures we have watched cost applicants a week. They are marked separately so you can tell which is which.
The documents
7 items| Document | Required form |
|---|---|
| Class 10 marksheet and certificatePractical note The name printed here is the one every later document is checked against. If it differs from your passport — an initial expanded, a middle name dropped — resolve that before you apply, not during counselling. | Notarised copy |
| Class 12 marksheet and certificatePractical note Physics, Chemistry and Biology must be visible as separate subjects with marks. A consolidated grade sheet without subject-wise marks is usually sent back. | Notarised copy |
| NEET scorecardPractical note Download and keep your own PDF as soon as results are published. The NTA portal does not stay open indefinitely, and a scorecard you cannot re-download is a problem you discover at the worst moment. | Original, for the year you are applying against |
| PassportPractical note No visa is needed for Nepal, but the passport is still the identity document the college and the Commission work from. If yours expires during the course, renew before you travel rather than from Nepal. | Valid, with adequate remaining validity |
| Medical fitness certificatePractical note Some colleges specify their own format or their own panel. Ask the college before paying for one. | From a registered medical practitioner |
| Character certificatePractical note Issued by your class 12 school or board. Request it while you are still in touch with the school — it is materially harder to obtain a year later. | From the last institution attended |
| Passport photographsPractical note Carry more than you think you need, in both physical and digital form. Several separate steps each consume a few. | Recent, passport specification |

Before you apply
3 items- Class 10 marksheet
- Class 12 marksheet
- NEET scorecard
These three decide whether you are eligible at all. Get the notarisation done early — it is the step that most often adds a week nobody planned for.
At application and counselling
3 items- Passport
- Passport photographs
- Character certificate
The Commission publishes the definitive list for each intake on its entrance portal, and that list governs over any list — including this one.
At admission
2 items- Medical fitness certificate
- Originals of everything above
Colleges verify originals against the copies submitted earlier. Never hand over originals permanently; they are shown, checked and returned.
